What ID documents are accepted to notarize a PS Form 1583?

You’ll need to provide one Photo ID and one Address ID (or a single document that serves both purposes). 

Photo ID (Pick one): 

• US state/territory/tribal driver’s license or non-driver’s ID 

• US passport 

• Uniformed Service ID (military) 

• US Access card 

• US University ID 

• Matricula consular 

• Nexus Card 

• Certificate of Naturalization 

• US Permanent Resident card 

Address ID (Pick one): 

• Current lease agreement 

• Mortgage or Deed of Trust 

• Home or vehicle insurance policy 

• Vehicle registration card 

• Voter registration card 

Note: Although the driver’s / nondriver’s ID is listed in as an option for both the Authorized Individual’s photo ID and address ID, it may be used for only one of the IDs (either photo ID or address ID), not for both.
