What ID documents can I use to notarize my PS Form 1583?

You need two different documents

One document proves who you are (Photo ID) and a second document proves where you live (Address ID). The same document can't be used for both, and the form won't let you choose the same ID type twice.

Accepted Photo IDs

Choose one of these documents as your Photo ID:

U.S. state, territory, or tribal driver's license or non-driver's ID card
Passport
Uniformed Service ID (U.S. military ID)
U.S. Access Card
U.S. university ID card
Matrícula Consular
NEXUS card
Certificate of Naturalization
U.S. Permanent Resident Card

Accepted Address IDs

Choose one of these documents as your Address ID. Its address must match the home address you enter on the form exactly.

Current lease agreement
Mortgage or deed of trust
Home or vehicle insurance policy
Vehicle registration card
Voter registration card
U.S. state, territory, or tribal driver's license or non-driver's ID card, but only if you didn't use it as your Photo ID

Documents that aren't accepted

Utility bills and bank statements can't be used as an Address ID.

If your insurance card doesn't show your address

Use your full insurance policy document instead. The policy document includes your address and is accepted in place of the card.

The easiest combination

Most people use a passport as their Photo ID and a driver's license as their Address ID.
