What happens if I don’t complete the PS Form 1583?

Without a completed PS Form 1583, we legally cannot accept any mail for you. This means: 

• Mail sent to your address will be returned to the sender 

• Your service may be paused until the form is submitted 

The good news: Once you submit the form, everything returns to normal.
